Frazier Rehab Institute is proud to be the host for the 2012 USQRA Division I & Division II National Championship Tournament on April 27 - 29, 2012. Our congratulations go out to all the teams who will qualify for the USQRA National Tournament and we welcome everyone for another fantastic year in Louisville, Kentucky.
 
As with any National Tournament there are a lot of details and we have tried to list below most of the important information to assist you in your preparations to attend. Additional information will be faxed or e-mailed to all teams as they confirm attendance at the Tournament.  If you have questions about the 2012 National Tournament which are not answered here please don’t hesitate to call us at (502) 582-7618 or (502) 582-7490.
 
ATTENDING TEAMS: Please complete and return a Team Registration and Roster Form (available on the USQRA website). Deadline for the submission of this form is Friday, April 6th. This form can either be faxed to (502) 582-7477 or e-mailed to jill.farmer@jhsmh.org or teri.kiper@jhsmh.org.   
 
Please be aware that your Team Registration and Roster Form should provide the current active members of your team (up to 15 ‘rostered’ players maximum) and support personnel (up to three maximum) per the USQRAguidelines for the Post Season Tournament Bid Package.
 
LODGING: We are pleased to announce that the Host Hotel for the Tournament will be the Galt House Hotel and Suites (www.galthouse.com) located at 140 North Fourth Street, Louisville, Kentucky 40202. This beautiful Hotel is located in downtown Louisville overlooking the Ohio River and adjacent to many fun and entertaining attractions. A group rate of $105.00 (plus a 15.01% Hotel Tax) per night for a standard room has been secured for this event. A variety of one and two bedroom suite options are also available at prices starting at $115.00 (plus a 15.01% Hotel Tax) per night depending on the option you choose. 
 
To reserve your rooms please go to the following website link: https://resweb.passkey.com/go/RUGBY12. This link will take you directly to the block of rooms reserved for this event. The website will guide you through the reservation process. Your credit card will be used only to secure the on-line reservation and will not be charged unless you indicate this is your method of payment when you check-in at the Galt House Hotel. You may also book your reservation by calling 1-800-626-2708 or (502) 589-5200. You must use a valid credit card and the Group ID# ‘979419’ to secure your reservation when booking a room by phone.  Please book your rooms by Friday, April 6th in order to secure the rate guaranteed for this event.
 
All accessible rooms within the Hotel have been reserved and will be available on a ‘first come-first serve’ basis. Remember the $105.00 rate will not be available after Friday, April 6thso it is important to reserve your rooms early. 
 
Parking is available on site in the attached, covered garage for all registered guests. The current rate is $12.00 per night for self-parking and $18.00 per night for valet parking (NOTE: this fee subject to change). A separate open parking lot is also available for large vans, buses and motor coaches. There is no charge for parking in this lot but space is limited and available on a first come first serve basis.
 
FLIGHTS: All participants should fly into Louisville International Airport (SDF). Please check with the USQRA Board for applicable airline or flight discounts which might be available to registered teams. 
 
AIRPORT TRANSPORTATION:Complimentary accessibletransportation will be provided to and from Louisville International Airport (SDF) for all participating teams, their support staff, AGM attendees, officials, classifiers and ‘Rugby Fans’.
 
To take advantage of this service all participating teams, their support staff, AGM attendees, officials, classifiers and ‘Rugby Fans’ must complete a “Transportation Request Form” (available on the USQRA website) and submit it by Friday, April 6th.  It can be faxed to (502) 582-7477 or e-mailed to karey.mcdowell@jhsmh.org or jill.farmer@jhsmh.org.
 
If you are planning to attend the AGM Meeting scheduled for Thursday, April 26th please complete an AGM Registration Form (available on the USQRA website) and make arrangements to arrive on Wednesday, April 25th. If we do not receive an AGM Registration Form from you by Friday, April 6th we will not be able to guarantee transportation will be available to you. This form can be faxed to (502) 582-7477 or e-mailed to karey.mcdowell@jhsmh.org or jill.farmer@jhsmh.org.
 
If you are a friend or family member planning to attend the 2012 USQRA National Championship Tournament we encourage you to purchase a “Rugby Fan Tournament Package” (available on the USQRA website). This package will provide you with a variety of options for the Tournament including airport transportation to and from the Host Hotel. If you do not purchase a “Rugby Fan Tournament Package” prior to Friday, April 6th you will need to secure alternate transportation from the airport to the Host Hotel. Other transportation options are listed below. 
 
All departing flights on Sunday, April 29th should be scheduled after 6:00PM. No transportation will be provided to the airport from the playing venue on Sunday, April 29th prior to 5:00pm after the final Championship game. If your flight is scheduled to depart prior to this time you will need to secure alternate transportation to the airport. 
 
Transportation to the airport will also be provided on Monday, April 30th by availability and special arrangement ONLY. All travelers must be prepared to leave the Hotel between 8:00am and 12:00pm or secure your alternate transportation to the airport.    For other transportation options please refer to the information listed below.
 
PLAYING VENUE:All games for the 2012 USQRA National Championship Tournament will be held in the Kentucky International Convention Center (KICC) located at 221 South Fourth Street, Louisville, Kentucky 40202. All Games will be played in Exhibit Hall 2-C located on the second floor of the KICC which can be accessed by elevator or escalator.   The KICC is located two blocks south of the Host Hotel and can be accessed through the covered skywalk as well as from street level. Signage will be on display to direct you to the playing venue both on the street and in the skywalk.  A downtown map has been included at the end of this information letter.
 
 
 
OTHER TRANSPORTATION OPTIONS: The Galt House has accessible airport transportation available to you with your reservation via Sandollar Limousine. To take advantage of this service you can go directly to their website, www.sandollarlimo.com or www.galthouseshuttle.com to give your flight information and details. The cost for this service is only $15.00 per person one way or discounted $25.00 round trip per person and can be attached to your room billing. Once at the hotel you can arrange your departing trip at the transportation desk directly at (502) 561-4011.
 
AGM: The USQRA Annual General Meeting (AGM) will take place on Thursday, April 26th at 9:00am at the Galt House Hotel (Meeting Room TBA). Each team affiliated with the USQRA is encouraged to send a representative to participate and vote on the important issues that pertain to the Sport and the Association.  If you are planning to attend the AGM Meeting you must complete an AGM Registration Form (available on the USQRA website) and make arrangements to arrive on Wednesday, April 25th.
 
PLEASE NOTE: If we do not receive an AGM Registration Form from you by Friday, April 6th we will not be able to guarantee transportation to and from the airport or lunch during the AGM Meeting. This form is available on the USQRA website and can be faxed to (502) 582-7477 or e-mailed to karey.mcdowell@jhsmh.org or jill.farmer@jhsmh.org.
 
COACHES MEETING: There will be a Coaches meeting on Thursday, 26th at the Galt House Hotel (Location and Time TBA). Coaches who cannot attend this meeting should have a Team Representative present in order to receive important information pertaining to the tournament itself. 
 
RUGBY FAN TOURNAMENT PACKAGES: Everyone wants to be a part of the action at the 2012 National Tournament. You can be an official “Rugby Fan’ by purchasing one of the available Tournament Packages ranging in price from $75.00 to $150.00 based on your individual or family/friend’s interests. To sign up as a ‘Rugby Fan’ please complete the registration form available on the USQRA website.  Rugby Fan Tournament Packages can be purchased now through the start of the National Tournament. This is a great way to see all the action while supporting the National Tournament. To register early please fax your completed form to (502) 582-7477 or mail it with your payment to Frazier Rehab Institute, Recreation Therapy Department-11th Floor, 220 Abraham Flexner Way, Louisville, Kentucky, 40202. Frazier Rehab Institute-Adapted Sports Fund is a non-profit 501(c) (3) organization as a part of the Jewish Hospital and St. Mary’s Foundation. All or a part of your purchase of the Rugby Fan Tournament Packages or Tournament Banquet Tickets may be tax deductible. Proof of our 501 (c) (3) status is available upon request. Receipts for Rugby Fan Tournament Packages or Tournament Banquet Tickets will also be provided upon request.
